New Patient Registration
Patients wanting to register with the practice must book an appointment with the receptionist. Patients will be advised at this stage that they should bring on their appointment photographic ID, proof of home address in the form of a utility bill along with their medical card.
Patients are advised if they don’t have a medical card they can complete a HS200 or a HS22X when attending for their registration medical.
Patients are registered with the practice and not an individual doctor. Patients have a right to express a preference but this may not always be possible.
You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
